STANFORD, Calif. For patients with stable coronary artery disease who have at least one narrowed blood vessel that compromises flow to the heart, medical therapy alone leads to a significantly higher risk of hospitalization and the urgent need for a coronary stent when compared with therapy that also includes initial placement of artery-opening stents.

Those are the findings of a study to be published online Aug. 28 in the New England Journal of Medicine that was designed to evaluate the benefits of using a diagnostic tool called fractional flow reserve, or FFR, to help determine the best course of treatment for fixing a narrowed artery.

"We believe there is a significant proportion of patients who benefit from stenting early on as opposed to receiving only medical therapy," said William Fearon, MD, associate professor of cardiovascular medicine at the Stanford University School of Medicine and co-principal investigator and senior author of the multi-center international trial called FAME 2. "For this group of patients who have significant ischemia [blood vessel narrowing that compromises flow to the heart muscle] based on assessment with FFR, the need for hospitalization and urgent revascularization is much higher and the pain relief is much less when only medical therapy is prescribed. People feel better and do better with FFR-guided placement of coronary stents up front in this setting."

The study's principal investigator is Bernard De Bruyne, MD, PhD, of Cardiovascular Center Aalst in Belgium.

The trial was halted early, on Jan. 15, because of the high rates of hospitalization and coronary stenting needed in the patients with significant ischemia who received only medical therapy. Some of those patients had suffered subsequent chest pain and heart attacks requiring urgent revascularization, which entails repairing damaged blood vessels with emergency stenting or heart bypass surgery.

Narrowing of the arteries caused by buildup of atherosclerotic plaque is common. About 40 percent of Americans over the age of 60 have one or more narrowings in the coronary arteries but no symptoms or stable symptoms, a condition known as stable coronary artery disease. Many can be treated with medical therapy alone, which may include aspirin or statins.

The idea for this trial grew out of the landmark COURAGE trial, presented in 2007, which found that stenting didn't provide any significant benefits over medical therapy for people with stable coronary artery disease. That trial relied solely on X-rays from coronary angiograms and noninvasive stress tests to determine which patients needed stenting.

The FAME 1 trial, which Fearon also helped coordinate, published in the NEJM in 2009 showed that doctors should go one step beyond the traditional method of relying solely on angiograms and use the additional method of measuring FFR to determine which arteries should or should not be stented for patients with coronary artery disease. Its findings demonstrated that FFR-guided stenting decreased a patient's chance of dying, having a heart attack or needing a repeat coronary revascularization, and reduced the number of stents necessary, thereby saving money.

The FAME 2 trial also used FFR, which involves inserting a coronary pressure guide wire into the artery to measure blood flow. In this study, it helped doctors pinpoint which patients would benefit from early stenting, as it identifies vessels with blood flow reduced to a dangerous level. As the study documented, these patients need stenting to prevent future adverse events.

"It's hard to know which patients are ischemic and which are not," said Morton Kern, MD, professor of cardiology at the University of California-Irvine. Kern was not directly involved with research on the study, but was a consultant on safety. "FFR is a very simple marker to help identify ischemia."

The cost is low, an additional $700 compared to the average cost of a stent, which is roughly $2,000. And the FFR technology is available to most physicians, he said.

The study included 888 patients with at least one stenosis one significantly narrowed artery causing ischemia that was identified with FFR. An artery was considered seriously narrowed if the blood pressure was 80 percent or less past the narrowing than the pressure in front of the narrowing. Patients were enrolled in 28 centers in Europe, the United States and Canada. Fifty patients were enrolled at Stanford University Medical Center and the Veterans Affairs Palo Alto Health Care System in the portion of the trial led by Fearon.

Trial participants were randomly assigned to either medical therapy alone or medical therapy combined with stenting.

Patient enrollment began May 15, 2010. By Jan. 15 of this year, 75 patients in the trial had experienced at least one "cardiac event," such as the need for hospitalization and an urgent stent or a heart attack. The rate of these events was 4.3 percent for patients randomized to stents plus medical therapy compared with 12.7 percent in patients assigned to medical therapy.

SAN JOSE, Calif. -- Apple scored a sweeping legal victory over Samsung Friday as a jury found the Korean company had copied critical features of the hugely popular iPhone and iPad and awarded the U.S. company $1.051 billion in damages.

The verdict -- which came much sooner than expected -- could lead to an outright ban on sales of key Samsung products and will likely solidify Apple's dominance of the exploding mobile computing market.

But the trial on Apple's home turf -- the world's largest and most influential technology market -- is considered the most important.

The fight began last year when Apple sued Samsung in multiple countries, accusing the South Korean company of slavishly copying the iPhone and iPad. Samsung countersued. Apple had sought more than $2.5 billion in damages from Samsung, which has disputed that figure.

The companies are rivals, but also have a $5 billion-plus supply relationship. Apple is Samsung's biggest customer for microprocessors and other parts central to Apple's devices.

The jury spent most of August in a packed federal courtroom in San Jose -- just miles from Apple's headquarters in Cupertino -- listening to testimony, examining evidence and watching lawyers from both sides joust about seven Apple patents, five Samsung patents, and damage claims.

Jurors received 100 pages of legal instructions from U.S. District Judge Lucy Koh Tuesday prior to hearing the closing arguments from attorneys.

Lawyers from both tech giants used their 25 hours each of trial time to present internal emails, draw testimony from designers and experts, and put on product demonstrations and mockups to convince the jury.

At times, their questions drew testimony that offered glimpses behind the corporate facade, such as profit margins on the iPhone and Samsung's sales figures in the United States.

From the beginning, Apple's tactic was to present what it thought was chronological evidence of Samsung copying its phone.

Juxtaposing pictures of phones from both companies and internal Samsung emails that specifically analyzed the features of the iPhone, Apple's attorneys accused Samsung of taking shortcuts after realizing it could not keep up.

Samsung's attorneys, on the other hand, maintained Apple had no sole right to geometric designs such as rectangles with rounded corners. They called Apple's damage claim "ridiculous" and urged the jury to consider that a verdict in favor of Apple could stifle competition and reduce choices for consumers.

The trial produced its share of drama and heated moments. Lawyers routinely bickered over legal matters in the jury's absence, filed rafts of paperwork to thwart each other's courtroom strategy, and sometimes even resorted to public relations tactics to make their views known.

Clinical trial targets breast cancer cells by injecting trastuzumab ...

In two to five percent of women with breast cancer, tumor cells migrate into the spinal fluid invading the tissue surrounding the brain and spinal cord called the meninges. While a rare complication, the condition is challenging because there is no agreed-upon standard of treatment, leaving little hope for patients affected. Northwestern Medicine- researchers are currently examining a novel approach to delivering an FDA approved drug that they hope will advance research for this type of cancer and lead to discoveries that may improve outcomes in the future.

As part of the clinical trial, the drug Trastuzumab is directly injected into the spinal fluid in hopes of stopping the growth of the cancer cells in patients with HER-2 positive breast cancer. Jeffrey Raizer, MD, co-director of the Northwestern Brain Tumor Institute (NBTI), is the principal investigator for trial which he developed. NBTI is part of the Robert H. Lurie Comprehensive Cancer Center of Northwestern University at Northwestern Memorial Hospital.

?When cancer spreads to the spinal fluid and tissues surrounding the brain, called leptomeningeal metastases (LM), there are very limited therapeutic options,? said Raizer, who is also medical director of neuro-oncology at Northwestern Memorial and associate professor of neurology at Northwestern University Feinberg School of Medicine. ?This rare condition typically occurs in later stages of cancer and often the benefits of treatment are small and may be counteracted by its side effects. For women with HER-2 positive breast cancer, they often have well-controlled disease in their body when this complication occurs.?

Raizer explains that drugs cannot easily penetrate from the blood stream into the spinal fluid because of the blood brain barrier, making the condition difficult to treat. In this trial, the antibody will be delivered directly into the spinal fluid using a device that is placed under the scalp called an Ommaya reservoir. A small catheter is inserted into a fluid-filled space allowing fluid to be removed and for the drug to be instilled into it.

This week marks our three-year anniversary (my favorite holiday). Since APW (of course) started as me writing about our planning and wedding, anniversary week always makes me particularly thoughtful. How does our wedding (now three years ago) relate to our marriage? How do relationships change and grow? So this week we wanted to talk about an idea that's integral to all long-term relationships: The Breaking Point.?That point where you come upon something that can?either?break you, or make you whole. This week, we're exploring how major events can enrich a relationship. And first up, we have A., writing about?becoming?a stepmom to a seven-year-old, at thirty-two.

Wedding planning traditionally involves shopping for silver, linen, and crystal. Flowers, candy, and jewelry are typically considered to be romantic gifts. Three weeks after moving in with my fianc?, he went out while I was at work and bought something for me: a pink car booster seat. I arrived home to find it in the foyer. He had picked it out with his seven-year-old daughter, my future stepdaughter, for use in my car when she rides with me.

I will admit right now that my first reaction was not positive. I am very glad to have my future stepdaughter, S., in my life. She's funny, smart, and eternally curious. I enjoy her company, and I feel like we're doing a pretty good job figuring out this whole living together as a family unit thing. But a car seat? in my car? My sporty single-girl truckette, now to be turned into a child-hauling grocery-getter?

A note of explanation on my relationship to my vehicle: My petite four-wheel-drive is practical, cute, and ready for adventure?words that I hope also describe me. It's not just a prized possession, paid off early through years of scrimping, but also one of my most personal spaces. Lacking a private study or home office, my car is the place where I can be by myself, crank up the music, and think. As in so many other areas, we have a gender role reversal in our relationship, as my fianc? couldn't care less about cars, while I tend to view mine as an integral part of my lifestyle (especially living in an area where public transit is not viable for our daily commutes). Suddenly, confronted by the brightly colored child safety device sitting in my new home, destined for a place in my motorized sanctuary, my head was spinning and the past few weeks of remarkably little moving-related tension seemed to fall away.

I had driven S. places before, but had always just borrowed the booster seat out of her dad's car. The purchase of a new car seat for my vehicle was precipitated by a commuting crunch. He's a professor at the university in the next town over, and is teaching an 8 a.m. class this summer. That means he has to leave early, and on the days we have S. with us, it would be impossible for him to get her to her summer day camp on time, plus make it to class. My work starts later and is near her camp, so it follows that I drop her off when she is with us. Thus it also follows that it made sense to buy an extra car seat for my vehicle.

Because I've always been cautious about the idea of parenthood or stepparenthood, we had, prior to moving in, tried to keep clear boundaries drawn around the work of parenting. He's the parent and I have been, in sequence, Daddy's friend, girlfriend, and now fianc?e. But I have never been a parental figure, to her or to anyone, up to this point. In fact, I would go so far as to say that being childless has been a defining point of my identity. In a society that encourages women to define themselves by their child-having status, and when you are of an age when many of your friends are having kids, it becomes easy to define yourself as "not-a-mom," and to incorporate non-motherhood into your self-concept.

The thing is, now that we've moved in together as a baby family, the lines are blurring, physically and emotionally. The physical manifestations of this blurriness are easy to point out: We eat groceries out of the same pantry. We share a mailbox. Possessions like towels, vacuum cleaners, flatware, and dishes are now held in common regardless of who originally bought them.

The emotional blurriness is harder to pin down. I do mundane parent-y things now, like asking S. to set the table and reminding her not to jump on the sofa. We also do fun things as a trio?like volunteering at the local food pantry together, taking walks that detour through the neighbors' lawn sprinklers, and planning Princess Bride?movie nights. S. and I have found common ground in a mutual love of glittery nail polish and history books. Slowly, the lines that once separated us into two distinct social units?him and her as family with me as a visitor?are starting to get fuzzy.

As I've discussed before on APW, the decision to marry my fianc??is also a deliberate decision to create a family with him and S. Like any engaged couple, we're creating a baby family?it just has more people in it than some, and thus is a little more complicated in the set-up.

As we prepared to move in together, we took inventory of our possessions and decided which items we could get rid of. Some things were easy?having two identical Ikea garlic presses meant we just had to put one in the charity box. Other things were harder. I gave up my favorite napping sofa. He gave up his 1960s sleeper couch and his bedroom dresser. Some negotiations over objects seemed little, but were actually kind of a big deal. I made the decision to use his fruit bowl in the kitchen, although I liked mine a bit better, because he was attached to it; he puts up with my enormous collection of FiestaWare dishes, to which I have my own unreasonable emotional attachment.

Feelings are a different matter. Difficult emotions aren't as easily discarded as a spare spatula. We can't transfer our feelings by putting them in a charity drop box. And we can't compromise on everything as easily as where we put our apples. So we have to talk about how we feel?ad nauseum, it sometimes feels like.

While walking and talking with my fianc?, I confessed that I felt a little weird about the car seat. The thing was, as long as I was just borrowing his car seat, I was also just borrowing the mantle of "parent" and all that goes with it. With the installation of the booster in my backseat, it felt like I was taking it on full-time, and that freaked me out.

My fianc? found this kind of amusing because, as he noted, our culture tells us that men are afraid of parenthood, marriage, and "settling down," while women push for it. Here we were, reenacting a clich?d scene in reverse. I was the one balking at the idea of any trappings of domesticity, from putting a car seat in my car, to having dinner on the table at a reasonable time every evening (a big change for someone who used to eat cereal for dinner when feeling too lazy to cook). Although he found this funny, I wasn't laughing?I was a little scared, as I felt like I was being pushed into a mold that didn't fit, needing to compromise who I had been for most of my adult life in order to accommodate our relationship and impending marriage.

Then, this morning, as I was driving S. to camp, her sitting in her pink-flowered car seat, I cranked up the radio. She said, "I know this song!" and started to sing along. I sang too.

She said, "My mom and dad always just listen to news in the morning."

"I like to listen to music, to get going," I said.

"Me too!" she responded.

So, we jammed to Top 40 the rest of the way to camp. I was doing things differently than her parents, and that was okay. We were having fun together, doing what I always do in my car in the mornings. I was being myself, and she was being herself. We were getting glimpses into each other's worlds as I listened to her talk about her friends, and she approved my choice of morning drivetime music.

Just as our stuff has gotten mixed up since moving in, our lives are all becoming ever more swirled together. Growing our baby family doesn't start when we say our vows next Spring?it starts now, through every small act of daily life. There are going to be concessions made, emotional fences dismantled, and self-concepts redefined. The process may get messy at times, just as our new home started as a mess of jumbled boxes. But as my future mother-in-law said, if we can blend our lives as well as we have blended our possessions, we'll be okay.

And I have to say, I think S. liked having her own seat in my car this morning. I certainly liked having her there. Next time I think I'll bring some Cyndi Lauper (one of her favorites and mine). Girls just want to have fun, after all, whether they're seven years old and getting a new stepmom, or thirty-two years old and becoming one.

The candidate will work closely with the Finance, Asset Management, and Construction teams to;

  • evaluate and recommend leasing strategies for each asset,
  • implement leasing strategies as determined by management and partners,
  • develop and evaluate market data,
  • solicit and attend new business pitches with the objective of growing the third-party leasing business,
  • work with the investment team to underwrite new acquisitions and disposition of assets
  • implementing the front line marketing focus for each asset under its guidance

The position will be based in the Washington DC office and will report directly to the Managing Director of Development and Leasing.

Overall Job Responsibilities

  • Leasing the assets assigned including the following activities;
    • Canvassing ?and cold calling for new tenants,
    • Meeting on site, touring, and working with existing tenants to finalize leases or lease extensions,
    • Attending networking events and conferences specific to retail and the retail brokerage community.
  • Develop a Leasing strategy and merchandising plan for each asset including all proforma assumptions;
  • Research and understand the market where investments have been made or are proposed in order to develop, evaluate, and implement the appropriate financial and operational objectives;?
  • Review and contribute to the annual business plan, leasing plan, project design, construction assumptions, and merchandise mix for each asset;
  • Interact with the asset management team on issues of new leases, renewals, and expansions;
  • Assist with investor and lender communications, as appropriate;
  • Assist in property dispositions, as needed; ?
  • Assist the new business team, finance director and CFO with analysis for new acquisitions and create income projections and lease up timelines, and marketing programs for new properties, as needed;
  • Prepare monthly reporting for partners and other team leaders using, Yardi, MRI or other such required software database programs.?
  • Monitor performance to ensure results are consistent with approved budgets and business plans or identify solutions to correct performance;

Relevant Experience: At least 5 years experience in commercial real estate leasing. The individual should have in-depth knowledge of the metropolitan DC commercial real estate market, trends in local growth markets, and a strong understanding of retail leasing tactics and processes. The individual will be expected to source new leasing activity and have the knowledge and skills to take the prospective tenant through the entire leasing process. Excellent computer and analytical skills, as well as knowledge of Yardi and MRI is preferred.

Compensation: Compensation will be commensurate with the experience of the individual and the requirements of the position. Compensation will include base salary, potential bonus, 401(K), and health and dental insurance.
